Our doctors work with you to identify and treat your healthcare issues. We can also provide advice about additional services to help you remain independent in your own home, if that’s what you wish for.
Annual health assessments for patients aged over 75 years
Our comprehensive annual health review for patients aged over 75 is an opportunity for us to talk to you about your health and wellbeing.
One of our practice nurses will conduct a comprehensive interview, preferably in your home, which takes around 1 hour. Afterwards, you attend your regular GP for a medical check-up to complete the assessment.
We’ll look at your physical ability, social support and psychological well-being. It’s all about preventative care – solving problems before they happen, and identifying things that can be done to help you live independently in your own home for as long as possible.
This health assessment is bulk-billed, so there is no cost to you.
Quality care at Residential Aged Care facilities
We offer care to all the residents at the Denmark Residential Aged Care Facility. If you have a regular doctor at Denmark Medical Centre, they will continue to provide care for you here.
We’ll work closely with you, your doctor and the staff at your facility to ensure your ongoing health needs are met.
