Established patients of our practice can be seen by their usual GP for the completion of a Firearms Authority Health Assessment.

This appointment is pre-paid and non-rebateable by Medicare, you will need to provide Section A of the WA Firearms Health Assessment Questionnaire to reception at the time of booking. Please bring parts B and C of the questionnaire with you to your appointment.

Your GP may want to send you for additional tests prior to completing Part B and C.  For example blood test, hearing or vision assessments.

Please note that the appointment is only for the medical assessment and not for any other medical problems or requests.
